Penile girth enhancement using hyaluronic acid filler has become an increasingly common, well-studied procedure. It is performed in-office, in roughly 60 minutes, with topical and injectable numbing for comfort, and minimal downtime.
We use the same FDA-approved fillers used in facial aesthetics, placed in the appropriate tissue planes to add even, natural-looking and natural-feeling girth. Results are visible immediately and last 12–18 months. Because the product is hyaluronic acid, it is also reversible if ever needed.
What makes our protocol different: most clinics only inject filler. We require neurotoxin (Botox) pretreatment two weeks before filler — this relaxes the penile smooth muscle, prevents post-treatment retraction that can shift filler and cause an uneven 'accordion' result, and improves blood flow. Blood always flows from high to low resistance, so relaxing the smooth muscle decreases resistance, increases inflow, and can improve both flaccid appearance (giving the look of added length) and erectile quality. This is why we view tox + filler as one comprehensive treatment, not two add-ons.

Same-day Botox pretreatment
If you choose to move forward, neurotoxin is administered the same day as your consult. Full payment for the enhancement is required at this visit.

Filler enhancement — 2 weeks later
Filler is placed 2 weeks after the tox so the neurotoxin is at peak effect. Even, layered placement — typically 6–10ml depending on goals.

- What is the consultation process like?

If you decide to move forward, we administer the Botox pretreatment the same day as your consult (full payment for the enhancement is required at that visit), then the filler enhancement is performed 2 weeks later once the tox is at peak effect.
- Why is Botox required before filler?
- Botox relaxes the penile smooth muscle. Without it, natural retraction can shift the filler and create an uneven, accordion-like look. Relaxing the smooth muscle also lowers vascular resistance — blood flows from high to low resistance, so inflow improves, which supports erectile quality and gives the appearance of added length even at rest. The 2-week wait ensures the tox is at peak effect before filler is placed.
- How is your approach different from other clinics?
- Most clinics only inject filler for girth. We add neurotoxin pretreatment as part of the standard protocol. The result is a more even, natural appearance, improved blood flow, support for erectile function and — for many men — the visual benefit of added length at rest from the relaxed smooth muscle. It's the most comprehensive enhancement protocol available.
- Can I do more than one treatment?
